SERVICES AVAILABLE: -treatment of chronic illnesses to include testing, care and medications for high blood pressure, cholesterol, diabetes, cancer screening and other common health problems. -Non-emergency conditions i.e. minor infections, wounds (non auto-accident or work related), immunizations -Screening and treatment or referral re sexually transmitted diseases -Pre-natal care: pregnancy test, blood tests, health plan (diet, weight, blood pressure, vitamins) and referral for late stage care. -Women’s health: pap smears, breast exams -Family planning -Enrollment in prescription Patient Assistance Programs -Risk Busters: a comprehensive health program for the treatment and control of diabetes, hypertension and high cholesterol. -Dental referrals for pain or infection only and for one visit only. GUIDELINES: Bandera County residents only; with no insurance, Medicare, Medicaid, CHIP ELIGIBILITY: Patients with income below 200% Fed. Poverty Guidelines (FPG) FEES: Qualified patients are asked to make a contribution for the services they receive, based on their income. Those with incomes less than 100% FPG are not asked to contribute, although contributions are ALWAYS welcome! OTHER: Clinic will be closed for Thanksgiving and Christmas weeks. INFORMATION: Walk-ins accepted but appointments are preferred. |
