Care Improvement Plus is a unique Medicare Advantage Special Needs plan for Medicare beneficiaries living with diabetes, heart failure, chronic obstructive pulmonary disease (COPD), and/or end stage renal disease (ESRD). Care Improvement Plus is a complete Medicare health plan combining medical coverage – hospital and doctors’ visits – with a Part D prescription drug benefit and additional care management services, such as nursing support, health education, and tools to help monitor and manage chronic conditions. Eligibility Requirements: Care Improvement Plus is available to individuals who meet the following criteria: •Entitled to Medicare benefits under Part A and are enrolled in Part B •Have been diagnosed with diabetes, heart failure, chronic obstructive pulmonary disease (COPD) and/or end stage renal disease (on dialysis). •Reside within the plan's service area: oArkansas (entire state) oGeorgia (entire state) oMissouri (entire state) oSouth Carolina (entire state) oTexas (entire state) o11 counties in Maryland (Baltimore City and Anne Arundel, Baltimore, Calvert, Carroll, Charles, Harford, Howard, Montgomery, Prince George’s and St. Mary’s counties) Cost of Services Care Improvement Plus offers seven plan options with premiums ranging from $19.00 per month to $397.00 per month. Beneficiaries with ESRD may only join the Silver Rx plan. Additional Comments: Care Improvement Plus offers more coverage and services than Original Medicare and some Medicare Advantage plans, including: • An open access provider network: Members may go to any Medicare-approved provider that will accept payment from Care Improvement Plus. No referrals are required. •Affordable Co-payments: With Care Improvement Plus, affordable co-payments for medical services and prescription drugs help keep costs down. •Vision Care benefits: Care Improvement Plus plans include benefits for routine eye exams and provide a yearly allowance for eyewear (lenses and frames). •Podiatry Benefits: Care Improvement Plus plans provide podiatry benefits with $0 co-payments, for up to six routine visits a year. •Coverage for preventive dental: Care Improvement Plus plans include coverage for preventive dental care services, include oral exams and cleanings. •Transportation to and from medical appointments: Care Improvement Plus plans provide transportation to and from medical appointments for plan-approved locations. The maximum number of trips depends on the plan selected. •Care Management Services: Members may consult with a nurse for questions about their treatment plan, symptoms, or medications. Members also receive regular coaching calls, health education materials, as well as monitoring tools for better self-care. |
